Required forms – please sign & return for registration.
Returning families register by mail (even if you have a new child starting)
New families register in person on Sept 2, noon-3pm or Sept 8, 9am-noon
- Volunteer contract & opportunities (2 pages)
- Library contract & volunteer opps (for your child to be able to take out books – 2 pages)
- NEW! Field trip permission form & policy (valid for the school year)
- Your child’s student record (printed packet only)
